Heartburn after meals or lying down, acid regurgitation, sometimes chronic cough, hoarseness or asthma-like symptoms.

Lifestyle measures

  • Weight loss if overweight
  • Avoid late evening meals
  • Raise the head of the bed
  • Limit alcohol, tobacco, fatty meals, chocolate and coffee in sensitive individuals

Medication

Proton pump inhibitors heal oesophagitis effectively but should be used at the lowest effective dose. Long-term use without indication has been linked to minor risks (B12 deficiency, fractures, kidney injury).

Alarm features

Difficulty swallowing, weight loss, anaemia, vomiting blood — these warrant prompt endoscopy.

Step-down therapy: many patients can reduce or stop PPIs after symptom control with lifestyle measures.
