What it is, in plain words
Vitamin B12 is needed to make red blood cells and to maintain the myelin sheath around nerves. It is absorbed in the last part of the small intestine only when bound to intrinsic factor produced by the stomach. Any break in that chain causes deficiency, and the nervous system can be damaged before the blood count changes.
Signs to look out for
- Fatigue, breathlessness, pallor and a smooth sore tongue
- Pins and needles in the feet and hands, in a stocking-glove pattern
- Unsteady walking, especially in the dark, from loss of position sense
- Memory problems, low mood and irritability
- Mild yellowing of the skin and eyes
Why it happens
Causes include pernicious anaemia, an autoimmune loss of intrinsic factor, stomach or bowel surgery, Crohn's disease and coeliac disease, long-term metformin or proton pump inhibitor use, and diets without animal products.
How doctors confirm it
Serum B12 is the first test, with methylmalonic acid or homocysteine when the level is borderline. A full blood count may show large red cells. Anti-intrinsic factor antibodies confirm pernicious anaemia. Folate, ferritin and thyroid are checked alongside.
Treatments and solutions
- Intramuscular hydroxocobalamin injections, loading doses then maintenance every two to three months, for pernicious anaemia or malabsorption, which is lifelong
- High-dose oral B12 for dietary deficiency, and it also works for many absorption problems at 1000 to 2000 micrograms daily
- Correcting folate and iron at the same time when they are also low
- Reviewing contributing medicines with your doctor
- Supplementation planning for vegans and after bariatric surgery, before symptoms appear
Living with it day to day
Neurological symptoms can improve substantially but only if treated early; after many months, some damage becomes permanent. Never treat with folic acid alone when B12 might be low.
