Not every cough warrants antibiotics, but some do urgently.

Sudden fever, productive cough, chest pain on breathing, and breathlessness. Older adults may present only with confusion or falls.
Warning signs
- Respiratory rate above 30/min
- Confusion
- Blood pressure below 90/60
- Oxygen saturation under 92%
- Inability to keep fluids down
Diagnosis
Chest imaging confirms consolidation. Blood tests and sputum culture guide antibiotic choice when severity warrants.
Prevention
Pneumococcal and influenza vaccination, smoking cessation and good oral hygiene reduce risk, especially in adults over 65 or with chronic illness.
Persistent fever beyond 3 days under antibiotics requires reassessment, not patience.
